Does insurance cover a professional Lice Treatment?
Lice Ladies professional treatment services, under our provider/Founder, are possibly reimbursable by many insurance plans, based on the insured member’s out of network benefits. We do not bill insurance. Payment is due at time of service.
Please contact your insurance company to determine the coverage. Your insurance company best answers questions about whether your lice treatments are covered by your insurance plan.
Upon request, at your appointment when you check out, Lice Ladies will provide you with the appropriate form which includes the diagnostic and procedure codes needed to submit a claim for reimbursement. Your pre-filled insurance form to file with your insurance provider will then be included with your discharge papers.
We also accept FSA and HSA debit cards.
You may also request a letter of medical necessity from the corporate or franchise owner in writing to the appropriate email address.
